Health Survey Completion
$20 reward for members who complete their annual Health Risk Assessment. Reward can be used for over-the-counter medicine not covered by Medicaid.
Reward Details
- Reward available once per program year.
- Must complete Parkland Community Health Plan's yearly Health Risk Assessment on the Member Portal.
- Reward points are awarded in approximately 3 months.
- Activity must be completed between September 1, 2025, and August 31, 2026 to earn this reward.
- Reward points can only be used to shop for items in the rewards catalog.
- Reward points cannot be used to purchase alcohol or tobacco. Reward points have no cash value and cannot be converted into cash.
